If you have received an OPHEN Professional Development Subsidy your are required to submit a report upon completion of the event and provide a brief update at a quarterly membership meeting, sharing key learnings from the educational experience.
Please complete the Subsidy Report Form and email a copy of relevant receipts to treasurer@ophen.ca
