Nomination Process:
The Leadership Award nomination form is accessed through the OPHEN Award Nomination website at https://ophen.ca/nominations.
Complete the nomination form by providing well-documented, detailed descriptions and specific examples of how the nominee demonstrates each selection criteria.
Select up to two referees who will each write a reference letter for the nominee. The reference letter(s) should illustrate how the nominee demonstrates one or more of the selection criteria.
Submit the nomination form and reference letter(s) to the Selection Committee via the OPHEN Award Nomination website between June 20, 2024 and October 18, 2024 at 5:00pm.
Members of the Selection Committee will review the submitted nomination forms and reference letters between October and November 2024. The award winner will be announced at the December 11, 2024, OPHEN membership meeting. Winners of the award receive recognition for their accomplishments, including a letter from the OPHEN Chair, an announcement on the OPHEN website, a free one-year membership for OPHEN, and an OPHEN tumbler.
If you have any questions about this nomination process, please contact the OPHEN Secretary at secretary@ophen.ca.
Submitting Reference Letters
Nominators are asked to submit up to two reference letters with the nomination form. The nominator and nominee cannot act as one of the referees. The referees should describe how the nominee's work has demonstrated one or more of the selection criteria. The reference letter must include the contact information for the referee. The referee(s) will only be contacted if two or more nominees are assigned the same score based on the selection criteria. The referee(s) will be asked to comment on an additional selection criterion not included in the nomination form - passion for the field of evaluation.
Before You Submit the Nomination Form
Consult with other people (even the nominee!) who can speak to the nominee's work and offer additional examples of how the nominee fulfills the selection criteria. This will ensure the Selection Committee is presented with a comprehensive view of the nominee.
Check to ensure you have provided well-documented, detailed descriptions and specific examples of how the nominee fulfills each selection criteria. The Selection Committee can only assess the nominee based on what information is included in the nomination form and reference letters.
Check that you have provided clear and well-written information about the nominee (e.g., complete sentences and not bullet points).
Consider whether you have made the nominee shine.
